Ninja Forms – CiviCRM

The Ninja Forms CiviCRM add-on connects your forms to your CiviCRM account without code. Add a CiviCRM Contact action to any form to create a new contact and capture contact type or subtype, email addresses, phones, IMs, websites and addresses.

You can link the contact to activity types and statuses, subscribe it to a group, add tags and notes, and register it to an Event ID. Each action can also check whether the person submitting the form already exists. It matches on email address only, name plus email address, or name plus physical address. On a match, the form updates the existing contact instead of adding a new one.

The connection to CiviCRM is automatic, and a built-in diagnostic panel shows its status.
