About This Plugin
This extension for Download Monitor requires users to fill in a Ninja Forms form before they gain access to a download. Download PDFs only after a form is submitted. The Ninja Forms plugin must be installed and activated.
An easy-to-use shortcode displays the form wherever you want. It only generates the form, so you build your own download pages. Entered data is stored in your Ninja Forms form like your other forms, and entries export to a CSV file.
Pair it with the Email Notification extension to get notified immediately when a form is submitted.
